Understanding CIDP

Chronic inflammatory demyelinating polyneuropathy, or CIDP, is a rare autoimmune condition in which the immune system damages the protective covering of the peripheral nerves. It affects roughly a few people in every 100,000, most often adults in middle and later life.

CIDP causes weakness, numbness and tingling in the arms and legs that develops over at least two months and can make walking, gripping and balance difficult. It is usually treated with immunoglobulin infusions, corticosteroids or plasma exchange, along with physical and occupational therapy.

Symptoms patients hope to ease

  • Burning, tingling or electric nerve pain in the hands and feet
  • Muscle cramps and aching
  • Trouble sleeping because of nerve pain
  • Fatigue
  • Anxiety and low mood that come with a long-term illness

What the research says

Not enough research. There is little good research on cannabis for this condition specifically. Patients usually use it for related symptoms such as pain, muscle spasms, nausea or poor sleep, where some evidence exists.

  • There is little research on medical cannabis specifically in CIDP, and no clinical trials have been done in this condition.
  • The 2017 National Academies review found substantial evidence that cannabis can modestly reduce chronic pain in adults, with many of those trials in neuropathic pain from other causes.
  • Small trials in diabetic and HIV-related neuropathy suggest some people get modest relief from burning nerve pain.
  • Cannabis has not been shown to reduce nerve inflammation or change the course of CIDP.

How doctors approach CIDP

  1. Doctors often consider an oral product or tincture for steady relief of nerve pain, especially in the evening.

  2. A balanced CBD and THC product is a common starting point, since THC appears to play a role in nerve pain relief.

  3. Vaporized cannabis may be considered for brief flare-ups of pain because it works quickly.

  4. Starting low and going slow is especially important if you already take nerve pain medicines that cause drowsiness.

Risks and precautions

  • Cannabis does not replace immunoglobulin, steroids or plasma exchange, which are what protect your nerves.
  • Weakness and numbness already affect balance in CIDP, and THC can make unsteadiness and falls more likely.
  • Combining cannabis with gabapentin, pregabalin, opioids or sleep medicines can cause extra drowsiness.
  • Steroids can affect mood and sleep, and THC can sometimes add to anxiety or mood changes.
  • Tell your neurologist about cannabis use, especially if your strength or sensation changes.
  • Not recommended during pregnancy or breastfeeding, and never drive while impaired.

What to bring to your evaluation

  • A CIDP diagnosis from a neurologist, ideally with nerve conduction study or EMG results.

  • Records of your treatments, such as immunoglobulin or steroid courses.

  • A list of nerve pain medicines you have tried and their side effects.

  • A description of how pain, numbness and weakness affect your daily life.

  • A government photo ID and proof that you live in your state.

How to get certified for CIDP

  1. Answer a few questions

Pick your state, tell us your symptoms and what you've already tried. Review your answers and contact you to schedule your video visit.
  2. Meet your doctor by video

Join from your phone or laptop. Have a photo ID ready. The doctor reviews your history and decides whether cannabis is right for you.

CIDP: 9 common questions

Does CIDP qualify for a medical marijuana card?

Yes, in many places. 1 state program name CIDP on the official list, and 14 more let the certifying provider decide. Either way, a provider licensed in your state has to evaluate you and agree that cannabis is appropriate.

Which states list CIDP as a qualifying condition?

As of our latest check of each state's official list: Illinois. Providers can also certify it in Alaska, California, Colorado, Delaware, Florida, Hawaii, Louisiana, Maine and 6 more, which don't use a fixed list. Lists change, so confirm on your state's page before you apply.

Can I get certified for CIDP online?

Yes, in 12 of those states — including Illinois, California, Delaware, Hawaii, Louisiana and 7 more — your first evaluation can happen by video with a provider licensed there. In 3 others, such as Alaska, Colorado and Florida, the first visit must be in person, and renewals can usually be done online.

Can cannabis help nerve pain from CIDP?

There are no studies in CIDP itself, but cannabis has modest evidence for nerve pain from other conditions. Some people with CIDP report less burning and tingling and better sleep. It is best viewed as an add-on for symptoms, not a treatment for the disease.

Can I use cannabis with gabapentin or pregabalin?

Many people do, but both cannabis and these medicines can cause drowsiness and dizziness. Together, the effect can be stronger than expected. Start with a very small amount, avoid driving, and tell the doctor who prescribes your nerve pain medicine.

Will cannabis interfere with my IVIG infusions?

There is no known interaction between cannabis and immunoglobulin treatment. Keep your infusion schedule exactly as planned. Let your neurologist know you use cannabis so they can consider it when reviewing your symptoms.

Is CBD alone enough for CIDP symptoms?

Some people start with CBD because it does not cause a high. Evidence for nerve pain is stronger for products that include some THC, though. Your certifying doctor can help you decide whether a balanced product makes sense.

Can cannabis make my weakness or balance worse?

THC can cause dizziness and slow your reactions, which matters when CIDP already affects your legs and balance. This can raise your risk of falls. Use it first at home, sitting down, and use any walking aids you normally need.

What is the best time of day to take cannabis for CIDP?

Many people use it in the evening, when nerve pain often feels worse and sleep is affected. This also keeps any THC effects away from daytime tasks like driving. Track your symptoms for a few weeks to see what timing works for you.
